Summary
Serves as a pharmaceutical caregiver by efficiently designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients. Maintains liaison relations with medical and nursing staff. Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.Essential Responsibilities and Duties
Coordinates with Supply Chain Management and handle borrowing, limits quantities, institutes and substitutes as appropriate and informs everyone accordingly.
Reviews the running-out list, critical items, out-of-stock items, and outlines course of action based on nature of item and professional judgment.
Checks received items for meeting the required hospital and SFDA standards.
Reviews product evaluation requests and prepares written communication as needed.
Periodicas reviews items that are not being used and takes appropriate corrective action. Maintains liaison with the drug information pharmacist in reviewing the formulary and other related matters.
Works with the Drug Information Center and Investigational Drugs in preparing cost saving report.
Keeps the pharmacy staff, managers, physicians, and other health professionals informed of significant shortages, receipts, new items through appropriate communications.
Reviews floor stock requests from Hospital departments and nursing units
Requests and facilitates the delivery of formulary/non-formulary medications for VIP/protocol patients.
Reviews generic substitution requests according to existing policy and procedure.
Coordinates with pharmacy areas and Supply Chain Management regarding the return of expired medications, with guarantee letters, to the Pharmaceutical Stores for replacement by the vendors in a timely manner. Education
Doctor or Master’s Degree in Clinical Pharmacy from an accredited college of Pharmacy or Pharm.D. Degree from accredited school in USA is required.Experience Required
General Pharmacy Practice Residency and one-year specialty residency accredited by ASHP, six (6) years of clinical experience in a hospital setting after completion of the last education program is required for candidates without residency training. For those with General Pharmacy Practice Residency only, four (4) years clinical experience is required.Other Requirements(Certificates)
For national or non-traditional Pharm.D. Programs, completion of at least one (1) year residency program is required. Board of Pharmacy Specialties (BPS) certification is required for all candidates
